How do you transfer deed to trust

My situation is this. What docs do you need to transfer equity to the trust.
I own a property and want to place it in trust. I then want to sell owner finance to my tennent. The way I understand most of the docs is to buy from seller and form a trust for him and then assign to investor. Is there a set of docs that will allow me to form my on trust. Please help.

First, you need to appoint a trustee.
Second, you need to create a declaration of trust.
Third, you need to file the deed from YOU to the Trust.
